About the position

The CAP/DA Case Manager plays a crucial role in providing comprehensive case management services to individuals in need of supportive services. This position is essential in ensuring that patients receive the necessary assistance to avoid institutionalization or to facilitate their discharge from an institution. The Case Manager will conduct formal assessments to determine eligibility for Medicaid or other payer sources, develop tailored care plans, and create financial budgets that align with the needs of the patients. The role requires a strong commitment to improving the health and well-being of the unique communities served by the organization, which boasts a diverse workforce of over 40,000 employees. In this position, the Case Manager will engage directly with patients and their families, performing patient interviews and fostering interpersonal relationships that reflect a positive image of the department and the hospital. The Case Manager will ensure that all interactions with physicians, community agencies, and other healthcare team members are conducted in a friendly, helpful, and courteous manner. This role is vital in coordinating care and ensuring that patients receive the support they need throughout their healthcare journey.

Responsibilities

  • Conduct formal assessments to determine Medicaid or other payer source eligibility.

  • Perform patient interviews and maintain positive interpersonal relationships with patients and families.

Requirements

  • Certified Social Worker or BSW required.
  • RN must meet NC Office of State personnel job specifications for a Public Health Nurse I or II or higher.
  • Three years of experience in a Social Work setting or three years in a Home Service Agency.
  • Experience in case management within an acute or long-term care setting.
  • Certification in Social Work and a current license to practice as an RN in the State of North Carolina.
  • Good physical, mental, and social assessment skills.
  • Understanding of patient care in the home environment for adult and geriatric populations.
  • Strong reading, mathematics, and communication skills.
  • Ability to work autonomously.
