About the position

The Care Coordination Specialist at RWJBarnabas Health plays a crucial role in the Population Health team, focusing on providing fully coordinated patient care. This position aims to improve the overall performance of value-based population health by ensuring patients receive appropriate care at the right time. The specialist is responsible for facilitating care components, closing gaps in care, and achieving quality performance targets, while serving as a liaison among patients, care management teams, physicians, and payers.

Responsibilities

  • Travel to assigned practices throughout NJ to meet with providers for EMR reviews and data abstraction.
  • Facilitate quality and utilization metrics data exchange between practices and the Population Health data team.
  • Liaise with practice management to share quality gaps reports and clinical opportunity reports.
  • Assist in EMR chart reviews, data abstraction, and tracking quality metrics for assigned programs.
  • Review charts to identify potential diagnosis codes and educate physicians on patient risk factors.
  • Manage internal and external relationships to ensure all needs are met.
  • Perform clinical point of care testing and other clinical assessment procedures as directed.
  • Develop partnerships with Clinically Integrated Network Practices and understand different EMR systems.
  • Serve as a patient advocate and assist in improving patient health outcomes.
  • Navigate insurance payors portals to meet quality gap guidelines.

Requirements

  • Associate's Degree or equivalent work experience
  • 4 years of relevant experience
  • Valid NJ Driver's License or ID card and dependable transportation
  • Medical Terminology knowledge
  • Understanding of HIPAA regulations

Nice-to-haves

  • Bachelor's Degree
  • MA or LPN with license to practice in New Jersey
  • Knowledge of population health
  • Experience in Epic or equivalent EMR system
  • Bilingual proficiency
