The Care Coordinator in this position at REAP (Resource, Entitlement, Advocacy Program) will be based at 1403-05 Madison Avenue, Manhattan. REAP plays a critical role in efforts to improve the health of our growing patient population. The organization helps reduce unnecessary admissions and emergency room visits by providing advocacy and assistance around benefits procurement and maintenance, as well as connecting patients to community resources. The Care Coordinator will join a dedicated staff of 13, including Care Coordinators, Social Workers, Administrative Assistants, and a Director. This role involves managing a caseload of patients and providing them with essential assistance in navigating various government benefit programs. Care Coordinators will conduct comprehensive screenings to assess patient eligibility for a range of government benefit programs, educate patients about the programs for which they qualify, and assist them in applying for, securing, and maintaining active coverage for these benefits. Collaboration is key in this role, as Care Coordinators will work closely with social workers and other internal and external agency staff to advocate on behalf of patients, helping them overcome barriers to accessing necessary care. REAP assists with a variety of government programs, including Medicaid, Medicare, Child Health Plus, Essential Plan, SNAP, and others. In addition to on-the-job training, Care Coordinators will participate in PEAK modules and online webinars, attend trainings at Medicaid, and complete a three-day training to become a Certified Application Counselor with the New York State of Health Marketplace. The position requires Care Coordinators to meet with patients during scheduled appointments while adhering to COVID safety guidelines. They will also contact relevant public benefit programs and community agencies as needed to address patient barriers to care. Documentation of their work in the REAP Salesforce database and, when appropriate, in a shared database with the Finance Department, is essential. Care Coordinators will have access to the electronic medical record as needed for checking insurance information. They will be assigned patient referrals and will complete comprehensive intakes for screening and appointment scheduling. The role also involves ongoing communication with hospital staff who refer patients to REAP, ensuring that referring staff are updated on the outcomes of their work with patients. Teamwork is a fundamental aspect of the role, as Care Coordinators will frequently communicate with other REAP staff about patient and program needs. Flexibility is crucial as REAP adapts to the growing demand for its services in a rapidly changing healthcare environment.
