Responsibilities

• Work collaboratively with a multidisciplinary care team to develop goals and plan interventions for high-risk patients.
• Provide care and disease management coordination.
• Identify patients with ongoing coordination needs and conduct targeted outreach.
• Conduct comprehensive clinical assessments covering various patient needs.
• Inform and work with patients and families regarding care coordination, education, and coaching.
• Monitor patient compliance with care plans and perform reassessments of patient progress.
• Serve as a liaison and advocate for patients and families.
• Assist in managing transitions of care across different settings.
• Identify barriers to care, facilitate solutions, and connect patients to community resources.

Requirements

• Graduate from an accredited school of professional nursing.
• Current state licensure as a Registered Nurse (RN).
• Basic Life Support (BLS) certification through the American Heart Association (AHA).
• Three to five years of nursing experience.
• Obtain cancer-specific certification or demonstrate ongoing qualifying education if working in oncology.
