About the position

The Health Home Plus Care Coordinator at EAC Network plays a vital role in supporting individuals with severe mental illness and complex needs. This full-time position involves coordinating care, engaging with clients, and collaborating with a multidisciplinary team to ensure comprehensive support and optimal health outcomes. The role offers a competitive hourly wage of $28 and a hybrid work schedule, allowing for both office and fieldwork responsibilities.

Responsibilities

• Provide outreach, assessment, and service planning for clients with severe mental illness.
• Develop and implement care plans, maintaining contact with clients and their support networks.
• Monitor client progress to achieve optimal health outcomes.
• Split time between office work and fieldwork, including home visits and hospital follow-ups.
• Collaborate with a multidisciplinary team to facilitate client engagement and address barriers to service utilization.
• Keep detailed records and track client activities, ensuring timely follow-up on appointments and treatments.

Requirements

• Master's degree in a human services field with one year of experience, or a bachelor's degree with two years of experience, or CASAC with two years of experience.
• Direct service experience with SMI, developmental disabilities, substance abuse, or children with SED.
• Strong organizational, time management, and communication skills.
• Proficiency in Microsoft Office and other relevant software.
• Valid NYS Driver's License and willingness to transport clients using own vehicle.

Nice-to-haves

• Experience linking individuals to essential services in a community setting.
• Two years as a Health Home care manager for the SMI or SED population.
