Richmond Behavioral Health has a full-time position available for a Care Coordinator at our North Campus location. This role involves intermediate professional work assessing client needs, developing, implementing, and monitoring service delivery and assistance plans, coordinating and monitoring services with other agencies, counseling and assisting clients, maintaining records and files, preparing reports, and related work as apparent or assigned. The Care Coordinator will be responsible for assessing client service needs, capabilities, and appropriateness for services, presenting options and services based upon a needs assessment, and developing an individual service plan of care in consensus with the client and their family. This includes reviewing and updating service plans as necessary. The Care Coordinator will also make referrals and linkages to appropriate agencies for services, coordinating client services and treatment with multiple service providers and agencies. Evaluating the quality of services provided and changes in the client's condition is crucial, as is counseling clients as necessary. The role requires evaluating the client's environment for safety, security, negative factors, and productivity, compiling and analyzing data relating to complaints, and identifying and working to resolve problems. Additionally, the Care Coordinator will plan and facilitate group sessions with clients participating in community activities and/or to promote recovery and educate clients. Providing motivational counseling to clients to facilitate or improve engagement in services that support recovery is also a key responsibility. The position serves as a liaison to public agencies, providing information regarding Authority programs and services. The Care Coordinator will attend inter-disciplinary team meetings to discuss decisions for client plans of care, provide or arrange transportation, and assist with discharge planning. Crisis intervention and prevention services are also part of the role, along with preparing a variety of reports and maintaining client charts and other records. Attendance at meetings, staffing, and conferences as they relate to client, staff, and program needs is expected, as well as attending court hearings and testifying in court when necessary. The Care Coordinator may also assist in curriculum development and resource tools.
