Responsibilities
- Follow a detailed plan to assess, implement, document, and update patient care plans.
- Work closely with the healthcare team to provide evidence-based care.
- Coordinate patient care and manage treatment goals and plans.
- Assess patients and families to meet their health needs.
- Help patients manage their health through education, visits, and phone calls.
- Research, evaluate, and recommend resources to meet both medical and non-medical needs of patients and families.
- Communicate across programs to coordinate services and act as a liaison to healthcare facilities.
Requirements
- Graduate of an Accredited Program
- Specialization in the field of nursing
- Two years of professional experience in a relevant field
- Licensed Vocational Nurse
