About the position

The Care Coordinator, RN at Bluestone Physician Services is responsible for managing the care of senior patients living with complex, chronic conditions and disabilities. This role involves working closely with multidisciplinary care teams to deliver tailored care that meets the unique needs of each patient. The position requires regular travel within the local area to conduct assessments and coordinate services, while primarily working from home. The Care Coordinator will ensure compliance with Department of Human Services and Center for Medicare/Medicaid Services requirements for members of the Minnesota Senior Health Options program.

Responsibilities

  • Manage member caseloads within the assigned geographic area.
  • Coordinate face-to-face visits with members.
  • Manage the Elderly Waiver program.
  • Conduct annual assessments including Personal Care Assistance assessments and customized living tools for members in Assisted Living.
  • Review current health needs, identify goals, and develop individualized care plans.
  • Connect members with community and state resources and services.
  • Complete required documentation accurately and timely.
  • Collaborate with medical care teams to ensure healthcare quality measures are met.
  • Support members during transitions of care and collaborate with care teams for safe discharges and follow-up plans.

Requirements

  • Current Minnesota Registered Nurse license or Minnesota Licensed Social Worker (new grads encouraged to apply).
  • One or more years of experience working with the geriatric population in case management/care coordination, Home Care, Nursing Home, TCU, or Assisted Living settings preferred.
  • Valid driver's license.
