About the position

The Care Coordinator 2 (Field Care Manager 2) at Humana Healthy Horizons in Indiana plays a crucial role in assessing and evaluating the needs of members to help them achieve optimal wellness. This position is part of the Indiana Medicaid program, specifically designed to support Hoosiers who wish to age at home, ensuring they have better access to services and improved health outcomes. The Care Coordinator will work closely with members and their families, guiding them to appropriate resources and facilitating interactions that promote their well-being.

Responsibilities

  • Employ various strategies to manage a member's physical, environmental, and psycho-social health issues.
  • Facilitate the development of a trusting relationship with each member to improve quality and continuity of care.
  • Coordinate all medical and non-medical services needed by the member, including functional, social, and environmental services.
  • Collaborate with the Service Coordinator, Transition Coordinator, and other care team staff to address identified member needs.
  • Coordinate with Medicare payers, Medicare Advantage plans, and providers to manage care for members eligible for Medicare.
  • Act as the primary point of contact for the Interdisciplinary Care Team (ICT) and ensure member needs are met.

Requirements

  • Licensed Registered Nurse (RN) in the state of Indiana without restrictions.
  • At least one year of experience in case management or similar healthcare services.
  • Intermediate to advanced computer skills, particularly with Microsoft Word, Excel, and Outlook.
  • Exceptional communication and interpersonal skills to build rapport with customers and stakeholders.
  • Strong critical thinking, organization, written and verbal communication, and problem-solving skills.
  • Ability to manage multiple priorities in a fast-paced environment.
  • Experience with electronic medical records and various electronic information applications.

Nice-to-haves

  • Bilingual (English/Spanish) or (English/Burmese).
  • Prior nursing home diversion or long-term care case management experience.
  • Experience with Medicare & Medicaid recipients.
  • Experience working with a geriatric population.
  • Knowledge of community health and social service agencies.
