About the position
The Care Coordinator at the Stonybrook Stabilization & Treatment Center is responsible for supporting clients with substance use disorders through the development and facilitation of educational and treatment groups, as well as aftercare planning. This role involves establishing a therapeutic relationship with clients, assessing their needs, and maintaining relevant documentation and reports. The position requires a compassionate approach to help clients navigate their recovery journey effectively.
Responsibilities
- Develop and facilitate educational and treatment groups for clients.
- Create and implement aftercare plans for clients post-treatment.
- Establish and maintain a counseling relationship with clients.
- Assess and analyze information pertinent to client needs and community concerns.
- Maintain relevant information and submit necessary reports.
Requirements
- At least two years of clinical experience in substance addiction treatment.
- High School Diploma or equivalent required; LADC/CADC preferred.
- Ability to establish and facilitate therapeutic rapport with clients.
- Strong observation skills for making treatment safety and community referrals.
- Strong written and verbal communication skills.
Nice-to-haves
- Bilingual skills in English and Spanish are preferred.
