About the position

Orlando Health is seeking a Care Coordinator SW for Discharge Support within the Corporate Care Management department. This full-time position focuses on outpatient care management and involves collaboration with the clinical team to identify patients who would benefit from care coordination services. The role includes assessing patients' risk factors, clinical utilization management, and preventative care services. Responsibilities

  • Ensure continuity and consistency of care across inpatient, emergency, and outpatient settings.
  • Develop effective working relationships with Patient and Family Counselors/Social Workers and UR nurses.
  • Monitor progress towards discharge plans and adjust as necessary based on patient condition or family needs.
  • Educate patients and families with chronic illnesses about evidence-based standards of care and self-management strategies.
  • Identify support needs for patients and families, develop action plans, and provide guidance.
  • Facilitate relationship building between patients and various healthcare settings.
  • Ensure patients have access to prescriptions, durable medical equipment, and other necessary services.
  • Contribute to team problem solving through communication, collaboration, and data collection.
  • Advocate for patients to optimize their healthcare needs, including safety and financial well-being.
  • Refer patients to educational resources regarding healthcare delivery and community services.
  • Utilize IT resources for registry reporting and maintenance of patient populations.
  • Participate in clinical outcome measurement and promote population health strategies.
  • Ensure patient safety and compliance with policies and procedures.
  • Provide clinical treatment interventions under supervision and facilitate psychosocial support groups.
  • Demonstrate knowledge of medical/legal issues and patient rights.

Requirements

  • Master's degree from an accredited school of Social Work, Mental Health, Psychology, or Marriage and Family Therapy.
  • Handle with Care (HWC) Certification required for Behavioral Health Unit.
  • Two years of direct clinical experience with an emphasis on the population to be served.
  • Successful completion of a Master's level internship may substitute for two years of experience.
