About the position

The Care Coordinator for Trinsic at Intermountain Health is responsible for providing clinical support and coordination related to value-based care (VBC) programs. This fully remote position involves working in an advisory capacity to support standards of care, assisting with scorecard initiatives, and conducting patient outreach to improve health outcomes and reduce barriers to care.

Responsibilities

• Provide clinical support and coordination for value-based care programs.
• Assist with scorecard initiatives by working with assigned care providers on program metrics.
• Conduct patient outreach and follow-up for high-risk patients with gaps in care.
• Consult with care teams to provide care coordination for preventative care and utilization mitigation.
• Identify appropriate resources for patients, including transportation, housing, and healthcare services.
• Plan interventions to help patients navigate the health system and reduce barriers to care.

Requirements

• Experience as a Medical Assistant, in Care Management, or in Primary Care Practices.
• Knowledge of Social Determinants of Health and Value-Based Care.
• Familiarity with Quality Metrics and Public Speaking.

Nice-to-haves

• Experience in auditing documentation, coding, and billing practices in primary care.
• Ability to work with payers on documentation requirements and education materials.
