About the position

The Care Coordinator position at DAWSON involves providing care coordination services for the Minnesota Army National Guard, focusing on both surge and steady-state mission requirements. The role includes tracking medical statuses, processing medical and personnel records, and supporting case managers in managing Soldiers' medical care. The coordinator will ensure compliance with various regulations and maintain accurate records while educating Soldiers on available resources to improve their medical readiness.

Responsibilities

  • Provide care coordination services for the Minnesota Army National Guard.
  • Track medical status of all assigned cases or Soldiers as required.
  • Receive, communicate, and process medical and personnel records submitted in various e-systems.
  • Scan administrative and medical documentation into HRR and ensure proper titling within one week of receipt.
  • Support Case Managers in preparing and submitting RCMC/ARCP cases for Soldiers with low-risk medical conditions.
  • Monitor and update various trackers with the medical and dental status of Soldiers.
  • Complete medical charting, case processing, and documentation for MNARNG Soldiers in required systems.
  • Update current status of cases, contact information, and suspense dates.
  • Forward relevant medical documents to military or civilian medical providers.
  • Maintain contacts for case updates and medical records requests.
  • Manage Soldiers' cases from identification of conditions to medical readiness or administrative separation.
  • Ensure accuracy of managed cases in administrative trackers and complete charting in eCase within 24 hours of action.
  • Request profile exams and coordinate Soldier treatment plans with medical facilities.
  • Attend PHA events as needed and ensure referred Soldiers are followed by the case management team.
  • Educate Soldiers on resources available to improve medical and dental readiness.
  • Maintain HIPAA standards and guidelines in all processes.

Requirements

  • Preferred experience in the Army or National Guard.
  • Proficient in Microsoft Office and general administrative duties.
