About The Position
Join a community that's changing mental healthcare for the better. The Care Coordinator position has primary responsibility for collaborating with the treatment team to facilitate the appropriate discharge and transfer of clients to the appropriate services and level of care. This position is responsible for participating in the treatment planning process that begins from the initial assessment and unfolds throughout the course of treatment in the program through collaboration with the client and the BHUCC treatment team. The case manager participates in interdisciplinary meetings, collaboration and communication with outside partners and resources, and with preparing clients for program transfer and discharge. Confirms delivery of all ordered medication(s) from pharmacy on shift worked. Responsible for writing summary on clients as assigned. Makes written or oral shift assignments and plans, organizes, and directs the nursing activities of the unit (when needed and assigned) to assure quality care delivery.
