About the position

The Care Coordinator position at Meridian Behavioral Healthcare is a vital role aimed at making a positive impact in the lives of individuals facing mental health challenges and substance use disorders. This position is based in Lake City, FL, and offers a competitive hourly wage of $20.19. As a Care Coordinator, you will be responsible for completing comprehensive assessments to evaluate the total needs of clients, ensuring that those with opioid use disorders receive appropriate care. You will develop service plans in collaboration with clients and therapists, linking them to necessary services and resources. This includes providing support for childcare services and maintaining contact with various staff and agencies to monitor client progress and assess the appropriateness of services provided. In addition to direct client interaction, you will serve as an advocate for clients, promoting community program development to meet their needs. Your role will involve conducting face-to-face visits to monitor client progress, preparing and maintaining accurate client records, and ensuring the quality of documentation in medical records. You will also be expected to engage in other duties as required to meet the goals and objectives of the program and department. Meridian Behavioral Healthcare is a private, non-profit organization with a long-standing commitment to the mental health community. Responsibilities

  • Complete comprehensive assessments to evaluate the total needs of clients.
  • Develop service plans with specific goals and objectives in collaboration with clients and therapists.
  • Link clients and families with services and resources identified in the service plan.
  • Provide advocacy for clients with community agencies and center programs.
  • Promote community program development to meet client needs.
  • Conduct face-to-face visits to monitor client progress.
  • Prepare and maintain client records, ensuring quality documentation of medical records.
  • Record all activities engaged on behalf of a client daily via progress notes.
  • Perform other duties as required to meet program and departmental goals.

Requirements

  • Bachelor's degree from an accredited university with major coursework in health-related fields, psychology, or social work.
  • 1 year of experience in care management with primary care, SMI, SUD, or similar populations.
  • Valid Florida Driver's License with a clean driving record that meets company insurance requirements.

Nice-to-haves

  • Experience in mental health or substance use disorder treatment.
  • Familiarity with community resources and services for clients.
  • Strong communication and interpersonal skills.
