About the position
The Care Coordinator position involves working with clients in their homes and communities, focusing on providing holistic, person-centered care. The role includes teaching, skill building, and assisting clients with various needs, while collaborating with other agencies to promote recovery and resilience. Care Coordinators will work with both child/youth and adult populations, adapting their approach based on individual strengths and needs.
Responsibilities
- Provide holistic, person-centered care emphasizing personal strengths and skill acquisition.
- Use interventions to develop interpersonal, family, community, and independent living skills.
- Facilitate recovery/resiliency through goal setting and resource identification.
- Collaborate with individuals on treatment goals and ensure active participation in their care.
- Provide education on managing mental illness and substance use disorders.
- Assist individuals in accessing necessary rehabilitative services and benefits.
- Ensure communication and coordination with service providers and other stakeholders.
- Support wellness and recovery goals, including healthy lifestyle changes.
- Develop natural supports and improve communication and problem-solving skills.
- Provide family education and support to enhance the individual's support system.
- Help individuals develop skills for job maintenance and school success.
- Support individuals in crisis situations and coordinate resources as needed.
- Complete necessary documentation in a timely manner.
Requirements
- A valid QMHP Certification license.
- A bachelor's degree in a human services field or equivalent experience.
- Experience in teaching or working in helping services.
Nice-to-haves
- Experience with crisis intervention.
