About the position

As a Care Coordinator at the Westchester Institute for Human Development (WIHD), you will play a vital role in supporting patients with intellectual and developmental disabilities. This position involves working closely with patients, families, and healthcare providers to ensure timely access to care and enhance the well-being of patients and their families. The role is part-time and offers a flexible schedule, allowing for a work-life balance while making a meaningful impact in the community.

Responsibilities

  • Support the health of patients enrolled in Chronic Care Management (CCM) and Behavioral Health Integration (BHI) programs by confirming appointments, tests, procedures, and forms are scheduled and up to date.
  • Communicate regularly with patients and/or caregivers.
  • Handle telephone calls, emails, patient portal messages, tasks, and faxes, addressing routine information requests or coordinating with appropriate staff.
  • Act as a liaison with external healthcare providers regarding patient care issues.
  • Facilitate referrals and appointments to internal and external clinical specialties and other agency programs and services.
  • Recruit and introduce new patients and caregivers to CCM and BHI programs and acquire informed patient consent.
  • Document patient notes in electronic health records as appropriate.
  • Assist patients of the Primary Care Clinic and other specialty clinics as needed.
  • Collaborate with other Care Coordinators to ensure timely completion of routine documentation for patients' access to necessary services.
  • Provide clinical assistance as a Medical Assistant to ensure clinics are fully staffed as needed.

Requirements

  • High School Diploma required; Associate's Degree or certification as a medical assistant, LPN, or related field preferred.
  • 2 years of administrative experience and 1 year in a healthcare setting.
  • Experience working with people with intellectual and developmental disabilities preferred.
  • Knowledge of Electronic Medical Record software and proficiency in Microsoft Word and Excel.
  • Strong attention to detail and a commitment to assisting people in accessing healthcare.

Nice-to-haves
