About the position

The Care Coordinator at WestCare Foundation plays a crucial role in advocating for clients, particularly children exposed to violence and their non-abusing parents. This position involves coordinating services, making referrals, and collaborating with various agencies to ensure continuity of care. The Care Coordinator is responsible for conducting needs assessments, preparing service plans, and maintaining compliance with regulatory standards while providing direct support to clients.

Responsibilities

  • Provide direct services to children exposed to violence and their non-abusing parent.
  • Coordinate services provided by ancillary agencies, including linkages and referrals.
  • Conduct needs assessments to identify client requirements.
  • Prepare service plan documents for clients.
  • Develop discharge plans in coordination with treatment team members.
  • Provide sessions related to case management and service plan development.
  • Maintain client charts in compliance with licensing, CARF, Medicaid, and other regulatory standards.
  • Attend and participate in project-related training, interdisciplinary team meetings, staff meetings, and community task forces.
  • Perform other duties as assigned by the Director/Supervisor.
  • Understand and apply federal, state, and local confidentiality rules and professional standards of conduct.

Requirements

  • Graduation from an accredited four-year college with a degree in Addiction Counseling, Criminal Justice, Social Work, Psychology, or a related field is required.
  • Two years of related work experience in Mental Health and/or Substance Abuse field preferred.
  • Certifications/Licensures: Should be CAP, CPRS, or CPMH eligible. If not certified, must have the ability to obtain the credential within 2 years of employment.
  • Must be CPR/First Aid certified within 6 months of employment and every two years thereafter.
  • Ability to meet requirements of Monroe County Sheriffs Office background check.
  • Ability to provide an Affidavit of Good Moral Character.
  • Clearance in Pre-Employment Drug Testing and Criminal Records Checks, as required.

Nice-to-haves

  • Preference will be given to candidates who are currently certified in CAP, CPRS, or CPMH.
