About the position

The Care Delivery Manager (RN) is responsible for overseeing the nursing standards of excellence and ensuring high-quality care delivery for the underserved geriatric population at the PACE Center. This role involves managing day-to-day operations, supporting interdisciplinary teams, and coordinating care to enhance participant satisfaction and health outcomes. The position requires a strong focus on compliance, staff development, and effective communication across various departments.

Responsibilities

  • Review and ensure effective development and execution of personalized care plans that result in improved satisfaction & outcomes for all participants.
  • Facilitate and complete the process of Interdisciplinary Team (IDT) meetings, Service Delivery Requests (SDR), and other meetings.
  • Serve as a manager on call during non-operational hours and responsible for follow-ups and documentation.
  • Run necessary Clinical Delivery reports and represent the Community Health Nurse (CHN) in IDT meetings when required.
  • Balance and allocate staffing and related resources throughout the site to ensure adequate team assignments and coverage.
  • Communicate with IDT and necessary parties to coordinate care consistent with participants' health care needs and goals.
  • Collaborate with clinicians to review UAS and ensure the accuracy of assessments.
  • Investigate and address all incidents accordingly, ensuring timely documentation of Level 2 investigations.
  • Ensure complete and accurate documentation for participants, including enrollment and clinical notes.
  • Monitor care management activities and implement changes to meet needs and goals.
  • Maintain up-to-date knowledge about current health-related issues and recommend improvements in services and policies.
  • Collaborate with HR in conducting interviews, coaching, and disciplinary actions as needed.
  • Assist in onboarding and professional development training of staff.
  • Evaluate the work performance and competency of direct reports, providing ongoing feedback and completing annual performance reviews.
  • Investigate and resolve employee grievances and complaints.

Requirements

  • Degree from an NLN accredited nursing school (RN, BSN required or at least 5 years' experience in health care setting).
  • Successful completion of an accredited Nursing Program is required.
  • Completed NYSED-approved infection control coursework with a minimum of 8.0 education contact hours.
  • Minimum of 2 years of administrative experience in a management capacity in a certified home health agency or similar settings.
  • Customer Service experience is required.
  • Minimum of 1-year experience working with the elderly.
  • Current active and unrestricted license and registration in New York State as a Registered Nurse.

Nice-to-haves

  • Case Management Certification recommended within 2 years of employment.
  • Bilingual preferred: Spanish, Korean, Chinese, & Russian are a plus.
