About the position

The Care Manager (CM) PRN at Christus Health is responsible for collaborating with patients, families, physicians, and multidisciplinary teams to ensure effective patient progression through the continuum of care. This role involves developing and managing individualized care plans from admission to discharge, enhancing patient satisfaction, safety, and continuity of care. The CM also addresses barriers to effective patient care and ensures compliance with organizational standards and regulatory requirements.

Responsibilities

  • Interviews patients/families to gather information about social, emotional, and financial factors impacting health status.
  • Develops and monitors the patient's plan of care in collaboration with CMII or CMIII.
  • Coordinates patient care progression throughout the continuum of care efficiently and cost-effectively.
  • Acts as a resource and advocate for patients regarding treatment decisions and end-of-life issues.
  • Monitors patient length of stay and collaborates with interdisciplinary team members to expedite discharge.
  • Identifies and escalates barriers to diagnostic or treatment progress in a timely manner.
  • Assesses needs for discharge planning and continuing care/resource support post-discharge.
  • Provides information and support to patients and families to access needed resources.
  • Ensures appropriate communication and updates are documented for continuity of care.
  • Participates in Multidisciplinary/Patient Care Progression Rounds.

Requirements

  • Master's degree in Social Work or Bachelor's degree in Nursing or related field.
  • Licenses such as LVN/LPN, RN, LBSW, LMSW, or LCSW in the state of employment are required.
  • Experience in a clinical or acute care setting is preferred.
  • BLS Certification preferred.
  • Strong verbal and written communication skills.

  • Experience in case management, utilization management, and discharge planning.
  • Knowledge of community resources and social determinants of health.
  • Ability to multitask and function in a fast-paced environment.
