Provides advocacy, coordination, and continuity of evidence-based care for individuals and families in need through a collaborative process of triage, assessment, planning, and communication. Participates as a member of a multidisciplinary treatment team. Reports to the unit/department supervisor. This is a great opportunity for Bachelor's Degree graduates in the field study of Psychology, Social Work, Human Services, Criminology, Behavioral Health, Sociology, or Counseling.
