We are currently seeking a Care Manager - Housing Specialist for our Program for Assertive Community Treatment (PACT). This role is crucial in delivering intensive, community-based mental health services to individuals with severe mental illnesses, particularly those within the schizophrenia spectrum. The PACT model emphasizes a multidisciplinary approach, where team members share a caseload and work collaboratively to address the complex needs of our clients, many of whom also face issues such as substance abuse, trauma, and homelessness. The PACT Housing Specialist will play a vital role in utilizing the SAMSHA Permanent Supportive Housing (PSH) Evidence Based Practice to assist clients in securing and maintaining safe, affordable housing. This position requires a proactive approach, as 75% of services will be delivered in the community through outreach efforts. The Housing Specialist will develop and maintain relationships with housing providers, serve as a liaison between clients and housing resources, and support clients in navigating housing-related challenges. This includes providing expertise on housing subsidy programs, resolving conflicts with landlords, and ensuring that clients have the necessary support to live independently. In addition to housing support, the Housing Specialist will collaborate with the PACT team to provide comprehensive treatment and rehabilitation services. This role involves participating in daily multidisciplinary meetings, engaging with clients and their families, and contributing to discharge planning efforts. The ideal candidate will be committed to fostering community integration for clients and will assist in collecting data for program evaluation. This position is not only about providing housing assistance but also about empowering clients to overcome barriers and achieve their personal goals.

Responsibilities

  • Provide outreach-based, consumer-centered treatment, rehabilitation, and support, with 75% of services delivered in the community.
  • Develop and maintain collaborative relationships with key housing providers and serve as the primary liaison between housing resources and the PACT team.
  • Identify safe, affordable housing resources and serve as the expert on eligibility criteria, application processes, and lease signings.
  • Serve as the PACT expert on housing subsidy programs, Section 8 housing, rental assistance programs, and other housing-related resources.
  • Support clients in resolving conflicts with neighbors or landlords, including issues related to maintenance, safety, and rent payment.
  • Track and delegate tasks and deadlines required to obtain or sustain housing for PACT clients.
  • Collaborate with PACT team members to provide treatment, rehabilitation, and housing support retention services.
  • Participate in daily multidisciplinary staff meetings and work with individuals and their families to become collaborative partners in the treatment process.
  • Participate in inpatient discharge planning to troubleshoot housing-related barriers.
  • Complete clinical paperwork in a timely, accurate, and legible manner using FBH format.
  • Assist in collecting and maintaining data as required by ASO and University of Washington.
  • Participate in peer review, supervision, and in-house educational meetings as indicated.

Requirements

  • Bachelor's degree in psychology, counseling, social work, or a related field from an accredited college or university.
  • Must apply for Agency Affiliation within 30 days of hire.
  • Must have reliable transportation, insurance, and an active driver's license, and be willing to use a personal vehicle for work.

Nice-to-haves

  • Two years of experience working with psychiatrically impaired populations.
  • Experience working with homeless individuals and knowledge of issues impacting this population.
  • Knowledge of housing resources, housing rights issues, and the continuum of care for homeless persons.
