About the position

The Care Manager - LP is responsible for providing proactive intervention and coordination of care to eligible Vaya Health members. This role involves assessing members' needs, developing care plans, and ensuring appropriate services are delivered across various health domains. The Care Manager - LP works collaboratively with members, their families, and a multidisciplinary team to address care gaps and support transitions between care settings. This position requires a valid RN license and a Master's degree in a relevant field, along with experience in mental health and care management.

Responsibilities

• Provide proactive intervention and coordination of care to eligible members.
• Conduct comprehensive bio-psycho-social assessments to identify members' needs.
• Develop and implement person-centered care plans in collaboration with the care team.
• Participate in interdisciplinary care team meetings and ongoing care management.
• Support clinical transition planning and track individuals discharged from facilities.
• Utilize Vaya's Care Management software for documentation and care coordination.
• Ensure compliance with HIPAA and other regulatory requirements.
• Engage with community stakeholders to address service gaps and improve care access.
• Monitor and evaluate the quality of care delivered by providers.

Requirements

• Master's degree in health, psychology, sociology, social work, nursing, or a relevant human services area.
• Valid RN license or other acceptable licenses for relevant fields.
• Two years of experience working directly with individuals with behavioral health conditions or long-term services and supports.
• Experience in care management and coordination of services.

Nice-to-haves

• Experience working with individuals with intellectual/developmental disabilities or traumatic brain injury.
• Knowledge of the NC Medicaid program and accreditation requirements.
