About the position
The LPN Care Manager is responsible for coordinating patient care and services, ensuring quality care through a collaborative process. This role involves working with patients, families, care teams, payers, and external entities to facilitate safe transitions from the facility to other care settings, adhering to applicable laws and Life Care standards.
Responsibilities
- Coordinate the patient's care and services.
- Promote quality care through monitoring and evaluating services based on patient needs.
- Work collaboratively with patients, families, and care teams to ensure safe transitions to other care settings.
- Generate clear and concise written communication.
- Maintain excellent organizational skills and manage time effectively.
- Support the organization's goals and strategic plans.
- Make independent decisions when necessary.
- Implement and interpret departmental programs, goals, and policies.
- Maintain professional relationships with associates and vendors.
- Ensure confidentiality of proprietary information.
- Adhere to company policies and compliance procedures.
- Exhibit excellent customer service and a positive attitude towards patients.
Requirements
- Nursing diploma (associate's or bachelor's degree in nursing).
- Currently licensed/registered in applicable State as a Licensed Practical Nurse (LPN).
- One year of clinical experience in a post-acute care setting preferred.
- Prior case management, utilization review, and discharge planning experience preferred.
- Certified Case Manager (CCM) or Board Certification in Nursing Case Management preferred.

- Ability to train and educate patients, families, and associates.
- Proficiency in relevant computer systems for data entry and retrieval.
