About the position

The Care Manager, MSW position at Providence is a vital role within the Care Management team at Mission Hospital Regional Medical Center in Mission Viejo, California. This position is part of a collaborative practice model that includes patients, nurses, social workers, physicians, and other practitioners, as well as caregivers and the community. The primary focus of the Care Manager is to facilitate care along a continuum by effectively coordinating resources and addressing the psychosocial needs of patients. This role requires excellent communication skills, both verbal and written, to ensure that all parties involved in a patient's care are informed and engaged in the process. As a Care Manager, you will be responsible for assessing patient needs, developing care plans, and coordinating services to ensure that patients receive the appropriate level of care. You will work closely with a multidisciplinary team to provide holistic care that addresses not only the medical needs of patients but also their emotional and social needs. This position is essential in helping patients navigate the healthcare system and access the resources they need to achieve optimal health outcomes. Responsibilities

  • Assess patient needs and develop individualized care plans.
  • Coordinate resources and services to facilitate patient care.
  • Communicate effectively with patients, families, and healthcare team members.
  • Address psychosocial needs of patients and provide support as needed.
  • Monitor patient progress and adjust care plans as necessary.
  • Collaborate with multidisciplinary teams to ensure comprehensive care delivery.

Requirements

  • Master's Degree in Social Work, Psychology, Counseling, or equivalent education/experience.
  • 5 years of relevant social work experience if holding a Master's Degree.
  • 6 months of clinical experience in an HMO, medical group, affiliated model, hospital, or medical/office/clinic setting.

Nice-to-haves

  • 3 years of experience in utilization management or case management.
  • Managed care experience.
