About the position

The Care Manager position involves providing clinical documentation, crisis intervention, and outreach services to clients in an outpatient setting. The role focuses on collaborating with consumers and treatment teams to enhance the quality of life for clients by coordinating necessary services and resources within the community.

Responsibilities

  • Completes assessments and assists other staff in completing assessments in a timely and comprehensive manner as required by CPC and regulatory agency policies.
  • Responsible for timely and accurate completion of all documentation and paperwork as per Agency policies and procedures.
  • Provides services to clients and their families, including education about their illness and medications, assistance with day-to-day problems, referrals, advocacy, and crisis intervention.
  • Reports to supervisor all client-related problems.
  • Provides linkages to natural community supports to access needed services to maximize independent living.
  • Coordinates with other agencies or professionals to provide services to the consumer.
  • Maintains knowledge of community resources, especially emergency services.
  • Participates in overall coordination and planning of services.
  • Works cooperatively with other staff and supervisor.
  • Seeks supervision as needed in a professional and competent fashion.

Requirements

  • BA Degree in Mental Health or related field.
  • Ability to communicate effectively and clearly in written and spoken English.

Nice-to-haves

  • BA Degree in Mental Health or related field with mental health experience and 3 years experience or Master's Degree in related field.
