About the position

The Registered Nurse Clinical Case Manager (RNCCM) plays a vital role in providing care management and coordination services for at-risk patients, particularly those with complex medical and psychosocial issues. This position is integral to the patient-centered medical home (PCMH) teams, working closely with physicians and providers to ensure comprehensive care delivery. The RNCCM is expected to apply population-based, chronic care management approaches and clinical protocols to enhance patient outcomes.

Responsibilities

  • Provide care management and coordination services for at-risk patients.
  • Implement population-based, chronic care management approaches.
  • Apply clinical protocols as part of the care management program.
  • Collaborate with PCMH teams led by physicians/providers to deliver patient-centered care.
  • Support patients with complex medical and psychosocial issues.

Requirements

  • Graduation from an accredited school of nursing.
  • Oregon Registered Nurse License upon hire.
  • 3 years of clinical experience as a Registered Nurse.

Nice-to-haves

  • Bachelor's Degree in Nursing.
  • 2 years of experience in home health, skilled nursing facility, or community health nursing.
  • 2 years of managed care experience and/or utilization management experience.
  • 2 years of care/case management experience.
