About the position

The Full-Time Care Manager RN Coach at Premise Health is responsible for developing personalized treatment plans and supporting a multidisciplinary team approach to health promotion and education. This role involves providing condition-specific self-care education, conducting coaching sessions, and collaborating with clinical team members to improve health outcomes for members with chronic conditions. The position emphasizes a proactive approach to healthcare, focusing on prevention and wellness in a supportive environment.

Responsibilities

• Develop personalized treatment plans using provider care plans and member preferences.
• Support a proactive, multidisciplinary team approach directed toward prevention, education, and health promotion.
• Provide condition specific self-care education to members enrolled in various condition management programs.
• Participate in outreach/follow-up phone calls and follow-up member visits.
• Document touchpoints and progress into the electronic medical record.
• Collaborate with clinical team members to arrange for further medical treatment when needed.
• Work with members to improve health outcomes and better management of chronic conditions.
• Deliver one-on-one and/or group coaching sessions in various health areas.
• Coordinate and conduct wellness programs in areas such as nutrition, stress management, and smoking cessation.
• Identify emergency situations and provide care within legal scope of practice.
• Facilitate Care Team meetings.

Requirements

• Currently licensed Registered Nurse in state of practice.
• Current Wellness Coach Certification from Wellcoaches® or equivalent certification (must be obtained within 24 months of hire).
• Current hands-on training in AHA or ARC Basic Life Support for healthcare providers required; Advanced Cardiac Life Support may also be required based on contract scope of service.
• Bachelor's degree (BSN) from a four-year college or university (preferred).
• At least three years' experience as a Registered Nurse caring for members with conditions such as Type 2 diabetes, asthma, heart disease, heart failure, or hypertension.
• At least three years experience as a Registered Nurse in an emergency room, occupational health clinic, or other ambulatory care clinic.
• Knowledge of Internet software, Spreadsheet software, and Word Processing software.
• Knowledge and experience with Electronic Medical Records preferred.
• Knowledge of workplace health and safety concepts and OSHA regulations preferred.
