About the position

The Care Manager RN at HCA Healthcare plays a vital role in enhancing patient care delivery through telephonic outreach and support for transitions of care, preventive services, and self-management. This position collaborates with primary care providers and interdisciplinary teams to meet quality improvement initiatives and ensure effective care management for patients in the Miami-Dade or Broward county area.

Responsibilities

  • Support the patient and primary care relationship through care delivery enhancement.
  • Conduct telephonic outreach to patients to guide them through transitions of care and preventive services.
  • Act as a subject matter expert in quality and value-based care programs such as MIPS and ACOs.
  • Develop professional relationships with primary care providers and practice managers to improve health and wellness.
  • Assist in the development of tools and workflows to meet quality initiatives.
  • Collaborate with interdisciplinary teams to achieve coordination of care and quality goals.
  • Conduct meetings with practice managers and staff to communicate program goals and provide education.
  • Monitor patient compliance with preventive screening and behavioral health management processes.
  • Contact patients after emergency department encounters to facilitate follow-up care.
  • Create care management action plans with patients and caregivers.
  • Identify and enroll eligible patients in chronic care management programs.
  • Facilitate specialty referrals as needed and document efforts accordingly.

Requirements

  • Graduate of an accredited college of nursing.
  • Current licensure as an RN in the state of residence.
  • Knowledge of physician office practice operations.
  • One year of experience in a physician practice is preferred.
