About the position

The Care Manager, RN, LVPP at Valley Health Partners is responsible for conducting assessments of patient and family needs, particularly for high-risk patients or those at risk for poor outcomes. This role involves collaboration with health professionals across the continuum of care and the formulation of a documented plan that ensures continuity and goal-setting. The aim is to equip individuals and their families to easily access resources and adopt healthy lifestyles. This position is supported through grant funding and will be subject to reapproval every two years.

Responsibilities

• Conduct assessments of patient/family needs for high-risk patients.
• Collaborate with health professionals across the continuum of care.
• Formulate a documented plan of care that ensures continuity and goal-setting.
• Equip individuals and their families to access resources and adopt healthy lifestyles.

Requirements

• Bachelor's Degree in Nursing or related field.
• 5 years' experience in case management, disease management, home health care, hospital, or physician office setting.
• 2 years' prior electronic medical record (EMR) experience.
• Registered Nurse licensure.

Nice-to-haves

• Master's Degree in Nursing, preferred.
• CACRN - Certified Ambulatory Care RN Ambulatory Care Manager within 3 years of hire.
• Care Coordination and Transition Management (CCCTM) certification or relevant certification approved by department leadership.
