About the position

The Care Manager RN is responsible for coordinating the overall interdisciplinary plan of care for patients from admission to discharge. This role acts as a liaison between patients, families, and healthcare personnel to ensure that necessary care is provided promptly and effectively. The position requires a blend of experience, judgment, and creativity to accomplish care goals.

Requirements

• Associates degree required; bachelor's degree preferred.
• Must be a registered nurse (RN).
• At least 2 years of clinical experience required.
• Familiarity with standard concepts, practices, and procedures within the nursing field.
