About the position

The Care Manager at Vaya Health is responsible for providing proactive intervention and coordination of care to eligible members, ensuring they receive appropriate assessments and services. This role involves working closely with members and their care teams to address care gaps and facilitate access to necessary resources across various health domains. The Care Manager will engage in comprehensive assessments, care planning, and collaboration with multidisciplinary teams to support members' health and well-being, particularly during transitions between care settings.

Responsibilities

• Provide proactive intervention and coordination of care to eligible members.
• Conduct comprehensive bio-psycho-social assessments to identify members' needs.
• Develop and implement person-centered care plans in collaboration with care teams.
• Facilitate care team meetings and ensure effective communication among stakeholders.
• Monitor and document care management tasks and member progress in the Care Management software platform.
• Support members in accessing necessary services and resources across health domains.
• Provide crisis intervention and transitional care management as needed.
• Ensure compliance with HIPAA and other regulatory requirements.

Requirements

• Bachelor's degree in a relevant field (health, psychology, sociology, social work, nursing).
• Two years of experience working directly with individuals with behavioral health conditions.
• Two years of experience in Long-term Services and Supports (LTSS) or Home Community Based Services (HCBS) coordination.
• Valid RN License if applicable, with experience in mental health care.

Nice-to-haves

• Experience working with individuals with intellectual/developmental disabilities (I/DD) or traumatic brain injury (TBI).
• Knowledge of the NC Medicaid program and NC Medicaid Transformation.
