About the position

The Care Manager at TheKey is responsible for providing comprehensive, concierge-based care to clients, ensuring their dignity, safety, and independence as they age at home. This part-time role involves working closely with clients and their families to deliver exceptional care experiences, supported by training and resources from TheKey.

Responsibilities

  • Provide personalized care management for clients in their homes.
  • Coordinate and oversee care plans to ensure client needs are met.
  • Communicate effectively with clients and their families regarding care options and progress.
  • Monitor client health and well-being, making adjustments to care plans as necessary.
  • Collaborate with healthcare professionals to ensure comprehensive care delivery.

Requirements

  • Experience in care management or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and manage time effectively.
  • Knowledge of aging and home care best practices.

Nice-to-haves

  • Certification in care management or related field.
  • Experience working with elderly clients.
  • Familiarity with healthcare regulations and compliance.
