About the position

The Care Manager is responsible for completing assessments and developing Personal Care Plans to assist members in achieving their health goals. This role involves coordinating care, maintaining accurate member records, and linking members to community programs and services, such as housing and self-management programs.

Responsibilities

  • Create and update personalized care plans for members.
  • Collaborate with members' families, friends, and social supports in developing treatment plans.
  • Identify and link the member with high-quality healthcare providers.
  • Interact with providers to facilitate members' treatments.
  • Ensure the members' treatment requirements are met.
  • Link members to services and programs to help meet needs.
  • Conduct at least the minimum required contacts, based upon member acuity to evaluate progress, promote continuity of care, and ensure improved health outcomes.
  • Maintain records of case management activities.

Requirements

  • Four-year degree in social sciences, social work, nursing field.
  • 2+ years of work experience working directly with individuals with behavioral health conditions.
  • Proficient with Healthcare Management Systems and Microsoft Office, Word and Excel.
  • Outstanding written and verbal communication skills.
  • Excellent organizational skills.
  • Ability to travel locally.
