About the position

The NKADD is seeking a qualified individual for the full-time position of Positive Choices Care Manager. This is NOT a remote position. The Care Manager is responsible for providing comprehensive care management services to Humana Medicare plan members suffering from addiction to alcohol or other drugs (AOD) and members with chronic health conditions which result in multiple emergency room visits. The Care Manager provides proactive outreach and ongoing care management to these members. This role is crucial in ensuring that members receive the necessary support and resources to manage their health conditions effectively. In this position, the Care Manager will conduct assessments, engage members in their own self-care, assist with the development of action plans, provide interventions, and refer members to substance use treatment and other community-based supports. The role involves conducting initial telephonic outreach to assess emergent needs, gathering demographic information, and introducing the Positive Choices program. Additionally, the Care Manager will perform home visits to administer assessments and develop community-based crisis plans with members, educating them about potential triggers for crises and engaging them in problem-solving to identify their supports and strengths. The Care Manager will also respond to significant event alerts regarding a member's hospitalization or ER visit, conducting in-person visits to review conditions that precipitated the event and coordinating referrals to appropriate treatment providers. This position requires a strong commitment to empowering members to follow up with medical appointments and adhere to their medical plans of care, as well as coordinating referrals to various treatment options and community-based recovery support groups.

Responsibilities

  • Provide comprehensive case management services to members.
  • Conduct assessments and engage members in their own self-care.
  • Assist with the development of action plans and provide interventions.
  • Refer members to substance use treatment and other community-based supports.
  • Conduct initial telephonic outreach to assess emergent needs and gather demographic information.
  • Introduce the Positive Choices program to members during outreach.
  • Conduct initial home visits and administer the ANSA assessment.
  • Perform additional evidenced-based assessments and interventions as indicated, including AWARE, PHQ-2, PHQ-9, and SBIRT.
  • Develop community-based crisis plans with members and educate them on potential triggers for crises.
  • Engage members in problem-solving to determine their supports and strengths.
  • Respond to significant event alerts regarding member hospitalizations or ER visits according to established guidelines.
  • Conduct in-person visits in facilities or at home to review conditions precipitating events.
  • Coordinate referrals to inpatient detox, intensive outpatient treatment, outpatient treatment, and community-based recovery support groups.
  • Empower members to follow up with medical appointments and adhere to their medical plans of care.

Requirements

  • Possess Kentucky Chemical Dependency Certification or be a Kentucky Licensed Social Worker.
  • Minimum of 3 years post-degree experience working with individuals with substance use, addiction, and behavioral health conditions.
  • Skilled in Motivational Interviewing techniques.
  • Good working knowledge and understanding of issues related to the mental and behavioral health population.
  • Knowledge of individual, family, social, and individual needs to engage in positive choices with addiction.
  • Tolerant, empathetic, and possess good interviewing capabilities.
  • Knowledge of community resources within Northern Kentucky is a plus.
  • A valid driver's license and reliable transportation for travel within the region for meetings and trainings and for home visits.
  • Proficient computer skills including Windows operating systems, MS Word, MS Excel, data entry, and the ability to learn new software applications as needed.
