About the position

The Care Manager at Clarvida is responsible for providing outreach, information-referral, and care management services within the ICCS/Enhanced Care Management program. This role focuses on delivering a comprehensive array of mental health and rehabilitation services, including counseling, crisis intervention, and skill training, while coordinating and monitoring the assessment and treatment planning process for clients. The position requires bilingual proficiency in Spanish and emphasizes a commitment to quality care and community integration.

Responsibilities

• Conducts intake and comprehensive evaluations of clients' life situations, problems, and needs.
• Prepares detailed plans of care with client participation and consent.
• Assists clients in implementing their plans of care by facilitating access to community resources and monitoring support services.
• Maintains regular contact with clients through home visits for personal support and intervention as necessary.
• Completes daily service progress notes and billing forms in compliance with company and government standards.
• Conducts outreach and provides information and referral services to individuals not requiring case management services.
• Ensures compliance with HIPAA and maintains professional boundaries with clients and staff.
• Develops and maintains professional relationships with agencies and service providers.
• Participates in management, staff, and client meetings and training activities as required.
• Meets monthly productivity requirements and documentation standards.

Requirements

• B.A./B.S. degree in human services, psychology, or a related field.
• One year of related experience in the field of behavioral health preferred.
• Valid California driver's license and auto insurance.
• Current CPR certification.
• Two years of equivalent prior case management experience.
• Strong interpersonal skills and excellent prioritization and organization skills.
• Good written and verbal communication skills.
