About the position

The Care Navigator - Child Welfare position at Centene focuses on developing, assessing, and coordinating care management activities tailored to the needs of members in the child welfare system. Responsibilities

  • Develop and assess care management activities based on member needs.
  • Contribute to the development of personalized care plans/service plans for members.
  • Educate members and their families/caregivers on services and benefit options.
  • Evaluate member needs, barriers to care, and available resources.
  • Recommend and facilitate plans for optimal healthcare outcomes.
  • Provide psychosocial and resource support to members and caregivers.
  • Coordinate between members and care provider teams to ensure timely access to services.
  • Monitor progress towards care plan goals and collaborate with healthcare providers for revisions.
  • Collect, document, and maintain member information and care management activities for compliance.
  • Perform on-site visits to assess member needs and collaborate with providers.
  • Provide education to care managers and members on healthcare procedures and benefits.

Requirements

  • Bachelor's degree in a related field.
  • 2 - 4 years of related experience in child welfare or foster care case management.
  • Current state's clinical license preferred if holding clinical licensure.

Nice-to-haves

  • Experience in pediatric case management.
  • Familiarity with local resources in Pasco, Hillsborough, Orange, Lee, and Sarasota counties.
