The Utah Department of Health & Human Services, Division of Correctional Health Services, is seeking a Care Navigator (Caseworker II) to join our team of skilled professionals. This position is crucial in providing evidence-based, individualized interventions aimed at stabilizing a wide range of mental health issues for offenders while they are in custody. The Care Navigator will work diligently to help individuals maintain their best level of functioning, ensuring that they receive the necessary support and resources to facilitate their rehabilitation and reintegration into society. The Care Navigator will be responsible for developing and implementing high-quality, evidence-based programs that focus on improving individual outcomes and enhancing community safety. This involves coordinating services that address various needs such as housing, transportation, employment, medical care, and mental health support. The role requires a commitment to promoting accountability and strengthening partnerships with community providers and other stakeholders, all while maintaining a positive work environment. The ideal candidate for this position will possess licensure as a Social Service Worker or Certified Substance Use Disorder Counselor, or be actively working towards obtaining such licensure. Experience in substance use disorder and mental health treatment, as well as case management principles, is essential. The Care Navigator will also be expected to teach educational classes and possess effective communication skills to interact with clients, families, and other professionals in the field. Knowledge of risk assessments and criminogenic risk factors, along with familiarity with medication treatment for substance use disorders, will be beneficial in this role.
