Curative has launched a health plan that is distinct from traditional offerings, placing Care Navigators at the forefront of this innovative approach. The Care Navigator plays a crucial role in the Curative member-oriented health plan by being assigned to specific member accounts. Their primary responsibility is to assist patients in navigating the complexities of healthcare, encouraging engagement in preventative care, and partnering with them to achieve their health goals. This position is essential in ensuring that members have positive experiences while seeking and receiving care, which is a cornerstone of Curative's mission. In this role, Care Navigators are tasked with empowering members to take charge of their health by providing them with the necessary tools and educational resources to adopt healthy lifestyle behaviors. They collaborate closely with Curative physicians and clinical staff to deliver personalized, preventative care tailored to each member's needs. By optimizing care and health outcomes, Care Navigators play a pivotal role in enhancing the overall patient experience. They are also responsible for supporting the operational aspects of the practice, which includes care coordination and developing metrics to track member engagement and identify those with unmet healthcare needs. A significant part of the Care Navigator's duties involves anticipating care interventions before serious health issues arise, thereby improving patient experiences. They educate patients about their health plans, address any questions, and follow up after health appointments to ensure that all needs are met. This position requires a thorough understanding of Curative's technology products, clinical programs, and policies, enabling effective communication with patients. Care Navigators utilize Electronic Health Records to manage patient information and are expected to greet patients warmly, providing tours of Curative facilities when necessary. This full-time position may require evening and weekend hours, with various shift options available throughout the week. The role adheres to US regulatory and Quality System requirements, ensuring compliance in all operations.

Responsibilities

  • Ensure assigned members have great experiences seeking and receiving care.
  • Encourage members to engage more with their health by providing tools and educational courses for healthy lifestyle behaviors.
  • Work with Curative physicians and clinical staff to administer personalized, preventative care.
  • Optimize care and health outcomes for members.
  • Support practice operations by assisting with care coordination.
  • Suggest and develop metrics to track member engagement and identify unmet healthcare needs.
  • Anticipate care interventions to improve patient experience before serious issues arise.
  • Educate patients on their health plan components and answer relevant questions.
  • Follow up with patients after health appointments to assist with follow-up steps, questions, and concerns.
  • Be knowledgeable about technology products, clinical programs, and policies, and communicate them effectively.
  • Use Electronic Health Records to manage and complete patient records.
  • Greet patients warmly and provide tours of Curative facilities.
  • Manage patient communications and respond to messages in a timely manner.

Requirements

  • 3+ years of experience in case management, care navigation, health plan, or medical office role.
  • At least 1 year of experience working in a clinical setting.
  • Fluent in English; Spanish fluency is a plus but not required.
  • Outstanding written and verbal communication skills in both languages.
  • Analytical mindset with strong problem-solving abilities.
  • Strong ability to use and learn new technology tools.
  • Strong interpersonal skills and ability to work well with others.
  • Exceptional attention to detail and organizational skills.
  • Willingness to continually self-educate and improve skills.
  • Capable of working independently with minimal supervision and as part of a team.
  • Skilled with standard computer programs.

Nice-to-haves

  • Understanding of medical terminology.
  • Experience working in healthcare, hospitality, or customer service.
  • Familiarity with phlebotomy and/or injections.
