About the position

The Care Transition Coordinator (CTC) at Solaris HealthCare North Naples is responsible for building and maintaining relationships with referral sources to enhance patient admissions and ensure quality care transitions. This role involves collaborating with clinical discharge coordinators, social workers, and physicians to facilitate smooth transitions for patients from hospitals to skilled nursing facilities. The CTC will also engage with patients and their families to provide information and support throughout the admission process, ensuring that all clinical needs are met and that patients are well-informed about their care options.

Responsibilities

  • Build and grow census and quality mix by developing referral relationships with existing customers and new business through account management systems.
  • Meet with clinical discharge coordinators, social workers, and physicians to facilitate patient admissions.
  • Communicate with patients and families prior to admission, providing facility contact information and answering questions about rehabilitation and skilled nursing.
  • Coordinate with nursing staff to ensure all necessary equipment and prescriptions are ready prior to patient arrival.
  • Maintain a list of patients being followed at each hospital and their referring sources for follow-up communication.
  • Conduct customer relations meetings to generate referrals and maintain relationships with existing referral sources.
  • Participate in community relations efforts, including presentations to educate professional referral sources and the community about services offered.
  • Complete administrative tasks such as maintaining records, tracking supply usage, and reporting observations regarding facility conditions.

Requirements

  • Bachelor's degree preferred or considerable experience in a comparable position.
  • 3-5 years of sales experience, preferably in healthcare services, products, or pharmaceuticals.
  • Ability to read, write, speak, and understand English.
  • Strong analytical and communication skills.
  • Ability to make independent decisions and demonstrate leadership.

Nice-to-haves

  • Experience in healthcare sales or marketing.
  • Familiarity with electronic referral systems and payer verification systems.
