About the position

The Care Transition Coordinator at InfuCare Rx is responsible for coordinating home infusion services for patients transitioning from hospital to home. This role involves indirect patient care, requiring specialized training to assess and address the home infusion needs of patients across various age groups. The coordinator acts as a liaison among healthcare providers, patients, and families, ensuring timely and appropriate referrals while utilizing community resources to support patient care. The position demands strong organizational skills, independent judgment, and effective communication to facilitate a smooth transition for patients requiring home infusion therapy.

Responsibilities

  • Coordinate referrals for home infusion services by obtaining comprehensive referral information and verifying benefits.
  • Collaborate with hospital discharge planners, physicians, and home care agencies to identify patient home infusion needs.
  • Review medical records and conduct interviews to assess patients' home infusion requirements.
  • Identify and report potential risk management issues related to home infusion services.
  • Formulate home infusion service referrals in collaboration with healthcare professionals and patients.
  • Provide patients with information regarding home infusion services and discuss insurance coverage and financial responsibilities.
  • Ensure accurate and complete referral information is provided prior to patient discharge from the hospital.
  • Promote a positive image of the hospital and home infusion company through effective communication and collaboration.
  • Participate in the coordination of care processes for home infusion patients.
  • Work with pharmacy, nursing, and quality departments to ensure safe and timely delivery of medications and supplies.

Requirements

  • 3-5 years of experience in infusion therapy, home care, or a hospital setting.
  • Practical knowledge of state and federal regulatory requirements related to infusion care.
  • Strong organizational skills and effective communication abilities, both written and verbal.
  • Ability to work independently with minimal supervision.
  • Familiarity with insurance industry requirements and patient education preferred.
  • Basic computer skills, including Microsoft Word and data entry.
  • Valid driver's license with a good driving record and auto insurance.

Nice-to-haves

  • Registered Dietitian license preferred.
  • Experience with JCAHO or ACHC regulations related to infusion care.
