About The Position

The Care Transitions Navigator - Behavioral Health position involves direct interaction with patients facing complex psychosocial, psychiatric, and substance use challenges at Baptist Health Richmond. This role emphasizes a holistic approach to patient care, addressing not only medical needs but also the social, emotional, and environmental factors affecting patient well-being. The Navigator provides comprehensive psychosocial consultations, discharge planning, and facilitates referrals to community resources to ensure effective transitions from hospital to post-hospital care.

Requirements

  • Master's Degree in Social Work (MSW) with a current KY License as a Certified Social Worker (CSW) or Licensed Clinical Social Worker (LCSW).
  • For CSW, supervision hours must be completed within 3 years of hire, and licensure must be obtained within 8 months of completing supervision hours.
  • Master's degree in counseling with associated licensure (LPCA, LPCC) may be considered in place of the social work degree/licensure.
  • Professionals must be in good standing with Kentucky Boards of Social Work and Professional Counseling.

Responsibilities

  • Provide comprehensive psychosocial consultations and support for patients and their families.
  • Facilitate discharge planning services in collaboration with the multidisciplinary treatment team.
  • Maintain an up-to-date knowledge of community-based services and resources.
  • Assist patients with resources and referrals based on their needs.
  • Address psychosocial concerns and social barriers to health for effective transitions from hospital to post-hospital care.
  • Act as an advisor and resource to fellow staff members.
  • Participate in meetings, staffing, and committees as requested.
  • Complete utilization review for direct admissions to Baptist facilities as needed.
