About the position
The Care Transitions Coordinator at Enhabit Home Health & Hospice plays a crucial role in assisting patients with navigating post-acute care. This position aims to create a positive impact on patient outcomes and enhance referral source satisfaction by developing patient-centered transition plans based on evidence-based clinical guidelines. The coordinator will also represent the organization in strategic relationships with health systems and healthcare providers.
Responsibilities
- Assist patients in navigating post-acute care to improve patient outcomes and referral source satisfaction.
- Integrate evidence-based clinical guidelines and protocols in developing patient-centered transition plans.
- Represent area branches in strategic relationships with health systems, hospitals, and healthcare providers.
Requirements
- Graduate of an approved school of nursing, therapy, or social work.
- Licensed in the state of practice.
- Two years of demonstrated field experience.
- Understanding of federal, state, and local laws governing Medicare certified home health and hospice operations.
- Basic technology skills, including operation of a mobile device.
Nice-to-haves
- Registered nurse or physical therapist preferred.
- Three years of field experience preferred.
- Previous experience in home health or healthcare sales preferred.
