About the position
The Caregiver position at Care Plus TLC involves providing personalized care to a client in their home. The role focuses on monitoring the patient's condition, assisting with daily living activities, and ensuring the safety and well-being of the client. Caregivers are expected to maintain effective communication with medical professionals and family members while adhering to health care protocols and documentation requirements.
Responsibilities
- Take care of 1 client in their home
- Monitor patient's condition and report changes/concerns to appropriate medical professionals and family members
- Provide Activities of Daily Living care as outlined in the Care Plan-bathing, dressing, feeding, etc.
- Understand First Aid and Emergency Care protocols
- Complete electronic documentation while on shift
- Provide medication reminders
- Perform trach and vent care
- Familiarity with Home Health Care, safety and sanitation standards and procedures
- Understand Confidentiality and Privacy Laws
- Exhibit excellent communication and interpersonal skills
- Knowledge of basic medical terminology
- Ability to undertake a variety of diverse care tasks and work as a team member
Requirements
- 1 year Home Health Care experience
- 1 year caregiver experience
- High School Diploma/Equivalency
