About the position

The Case Management Assistant (CMA) at AdventHealth Shawnee Mission plays a crucial role in supporting the social workers and case managers in their discharge planning responsibilities. This full-time position operates during the day shift, Monday through Friday, and involves a variety of tasks aimed at ensuring a smooth transition for patients from the hospital to their next level of care. The CMA is responsible for responding to resource requests for case management assistance from physician offices and community agencies, arranging transportation for patient discharges, and ordering Durable Medical Equipment (DME) as needed. Additionally, the CMA communicates with nursing units regarding the status and timing of patient discharges, ensuring that all necessary documentation is completed in a timely manner according to regulatory requirements. The CMA will also meet with patients and their families to assist with follow-up appointments and provide necessary information regarding their discharge. This role requires a high level of organization and prioritization skills, as the CMA must manage multiple tasks efficiently while maintaining a focus on patient care. The position involves cross-training with the Intake Planner to provide assistance and coverage when necessary, and participation in department staff meetings and performance improvement activities is expected. The CMA is expected to demonstrate a commitment to teamwork, honesty, and positive outcomes, while also supporting the mission and values of Shawnee Mission Health. Overall, the Case Management Assistant is integral to the hospital's continuum of care, working collaboratively with staff to facilitate patient discharges and ensure that patients receive the resources they need for a successful recovery.

Responsibilities

  • Attend in-service programs of continuing education as appropriate for job performance.
  • Respond to resource requests for case management assistance from physician offices and other community agencies.
  • Arrange transportation for patient discharges.
  • Order Durable Medical Equipment as indicated.
  • Transmit medical record information to nursing homes, rehab facilities, LTAC, home health agencies and hospice organizations for purposes of patient placement and discharge.
  • Contact various community agencies to obtain available resource information for use by the case manager and/or social worker.
  • Communicate with nursing units on status and timing of patient discharges and facilitate completion of patient discharge information as indicated.
  • Meet with the patient and family as needed in order to carry out assigned tasks.
  • Assist in ordering of department supplies and in providing phone coverage as assigned.
  • Assist patients with making follow-up physician and treatment appointments as assigned.
  • Deliver the CMS 2nd Important Message letter and process according to policy.
  • Acquire/track face to face forms.
  • Perform follow-up phone calls to patients as assigned.
  • Document in the patient's medical record timely and per regulatory requirements.
  • Cross-train with the Intake Planner to provide assistance and coverage when necessary.
  • Attend and participate constructively in department staff meetings.
  • Forward requests for clinical reviews left on department voice mail to appropriate staff members.
  • Participate in department PI activities, including preparation of various reports out of existing programs.
  • Assist with data collection and reporting as assigned.
  • Work collaboratively with staff to move patients efficiently through the hospital continuum of care.
  • Assist in tracking and documentation of avoidable days.
  • Identify cases with exceptional discharge delays and collaborate with case manager and Manager/Administrative Director for solutions and direction.
  • Recognize and manage risks, resolve issues and problems as soon as they arise, and report and document adverse events and reportable conditions.
  • Support Shawnee Mission Health's mission and values verbally and behaviorally.
  • Demonstrate proficiency in the use of the electronic medical record and programs specific to the CMA role.
  • Rotate weekend and holiday on-site coverage as assigned.

Requirements

  • Bachelor's Degree
  • 1 Year of Position-Related Experience
  • Kansas Licensed Bachelor Social Worker OR Kansas Licensed Practical Nurse OR Licensed in a Healthcare Related Field
