About The Position

The Case Management Coordinator for Assertive Community Treatment (ACT) is a vital community-based role focused on providing comprehensive support to individuals with mental health needs. This position is designed for passionate individuals who are eager to make a significant difference in the lives of those they serve. The coordinator will be responsible for assessing and evaluating the needs of ACT individuals, maintaining ongoing contact with their caseload, and developing personalized goals and program plans to address identified needs. This role requires a strong commitment to advocacy, as the coordinator will work to secure necessary community-based services and resources for individuals, families, and guardians. In addition to direct case management, the coordinator will monitor and evaluate living situations, school placements, and program placements to ensure consistency in treatment and progress. They will also assist in commitment procedures when necessary and participate in daily team meetings to discuss challenges and strategize solutions. Collaboration with the ACT psychiatrist is crucial, as the coordinator will attend meetings at least twice a week to ensure continuity of care and assist individuals with medication compliance. The role also involves developing treatment plans that are individualized and person-centered, reflecting the unique needs of each individual. The Case Management Coordinator will utilize evidence-based practices and cutting-edge technology to implement integrated strength-based assessments, evaluate progress, and provide crisis intervention as needed. This position requires a deep understanding of psychiatric and substance use disorders, as well as the ability to identify triggers for relapse and develop effective prevention plans. The coordinator will play a key role in fostering a supportive and inclusive environment, ensuring that all individuals receive the care and resources they need to thrive.
