About the position

The Case Management Extender at Vibra Healthcare plays a crucial role in providing quality, cost-effective care to patients recovering from serious illnesses or injuries. This position involves collaborating with healthcare teams to monitor and evaluate patient needs, ensuring optimal treatment outcomes and comprehensive discharge planning in a specialized hospital environment.

Responsibilities

  • Participate in a collaborative process to monitor and evaluate options and services to meet individual health needs.
  • Provide quality, cost-effective care utilizing selective benefits and alternative services best suited for the patient.
  • Ensure appropriate sequencing of treatment goals and implementation of a comprehensive discharge plan.
  • Demonstrate knowledge and skills necessary to provide case management services.

Requirements

  • Current, valid, and active license to practice as a Licensed Practical/Vocational Nurse or a Bachelor's or Master's degree in Social Work (BSW or MSW) in the state of employment required.
  • Current BLS certification from a Vibra-approved vendor required.
  • Experience in a rehabilitation or acute care setting preferred.
  • Proficient in collecting and retrieving material from medical records.
  • Knowledge of regulatory standards and compliance requirements.
  • Strong organizational, prioritizing and analytical skills.
  • Ability to make independent decisions when circumstances warrant.
  • Working knowledge of computer and software applications used in job functions.
