About the position

The Case Management Site Supervisor is responsible for overseeing staff and ensuring that the goals and objectives of the programs are met. This role involves supervising case managers, providing training, and ensuring effective service delivery to clients and families.

Responsibilities

• Supervises and evaluates Case Managers.
• Ensures effective social and health-related services are provided to clients and families through weekly supervisory sessions and monitoring of files.
• Ensures completion of screenings, assessments, treatment plans, and referral processes according to policies and procedures.
• Provides backup case management services.
• Develops, implements, or provides training for Case Managers.
• Ensures maintenance of case management records with appropriate reviews of services, clients, and any other required paperwork.
• Assigns cases based upon Tier levels to the appropriate Case Managers.
• Completes performance evaluations on assigned staff.
• Assists in the interview process and selection of personnel.
• Establishes and maintains public relations between the programs outside the agencies and the general community.
• Performs other related duties that are deemed necessary to carry out the job efficiently.

Requirements

• Master's degree in Social Work or related field from an accredited college or university.
• At least 3-5 years of full-time, paid experience providing direct services and case management in human services, social services, or health services.
• Extensive knowledge of community resources and service providers.
• Knowledge of local social service system preferred.
• Ability to work within and relate to a multicultural environment.
