Responsibilities

• Provide case management services to patients in a healthcare setting.
• Coordinate patient care and ensure effective communication among healthcare providers.
• Assess patient needs and develop individualized care plans.
• Monitor patient progress and adjust care plans as necessary.
• Educate patients and their families about health management and resources.

Requirements

• 1+ year of recent experience in case management nursing specialty and setting.
• Valid nursing license as required by the state of Virginia.
• Valid Basic Life Support (BLS) certification from the American Heart Association (AHA).
• Valid Advanced Cardiac Life Support (ACLS) certification through AHA recommended and may be required.
• Specialty-related certifications are preferred and may be required.
