About the position

The Case Manager is responsible for providing resources and referrals to families, conducting home visits, and advocating for families within their team assignment. This role involves maintaining documentation for assigned cases, completing agency assessments, and providing outreach services that employ prevention and intervention techniques. The Case Manager will also provide rotating emergency coverage to support clients in need.

Responsibilities

  • Conduct and schedule home visits with clients to monitor family functioning and stability.
  • Build and maintain trust and rapport with families.
  • Report incidents of child abuse and neglect and assist families in modifying behaviors or conditions.
  • Identify and articulate critical issues confronting families and determine appropriate actions.
  • Regularly review data systems to ensure proper documentation of interactions during home visits.
  • Investigate community resources to meet client needs and maintain an up-to-date listing of available programs.
  • Provide housing relocation assistance and ensure accommodations for homeless clients.
  • Maintain working relationships with outside agencies to advocate for client services.
  • Provide emergency coverage to support clients in critical situations.

Requirements

  • Bachelor's degree in Social Work, Education, Psychology, or related field.
  • 2 years of experience in community-based social services or case management.
  • Knowledge of community resources and services.
  • Excellent oral, written, and interpersonal skills.

Nice-to-haves

  • Experience in child development, family relations, and behavioral health.
